The actual pages of the booklet were not scanned in order, they were scanned in pairs relating to the structure of the booklet.  
The last page appears with the first page and pages 16 and 17 create the centerfold.

The original scans were close to 20 MB and a little too big for me to post.   I have, hopefully, scaled them down to a more manageable size.   If you have trouble with them, let me know.
